Advance Directives

A written Advance Directive, such as a living will or healthcare proxy, will be used to guide the decision-making of the care you receive if you should ever become unable to communicate your choices about your medical care.

If you have a written Advance Directive, it is important to provide a copy to the hospital, your family and your doctor.
If you do not have an Advance Directive and would like more information, or desire assistance preparing an Advance Directive, please inform your healthcare provider or MCh Social Services. Please ask your nurse to contact the Social Services Department or call (402) 426-1220.
