Advance Directives
A written Advance Directive, such as a living will or healthcare
proxy, will be used to guide the decision-making of the
care you receive if you should ever become unable to communicate
your choices about your medical care.
If you have a written Advance Directive, it is important
to provide a copy to the hospital, your family and your
doctor.
If you do not have an Advance Directive and would like more
information, or desire assistance preparing an Advance Directive,
